Abstract

Background. Diabetic retinopathy (DR) is increasingly recognized as a heterogeneous neurovascular disorder, yet preclinical models often fail to distinguish between the distinct pathophysiological clusters of type 2 diabetes. Recent clinical evidence demonstrates that patients with different metabolic phenotypes, specifically Severe Insulin-Resistant Diabetes (SIRD) versus Severe Insulin-Deficient Diabetes (SIDD), exhibit divergent patterns of retinal complications. However, the specific cytomorphometric signatures that distinguish these phenotypes at the cellular level remain poorly characterized.

Aim: to determine whether specific systemic metabolic phenotypes, SIRD versus SIDD, induce distinguishable cytomorphometric signatures of retinal neurodegeneration in experimental models.

Materials and Methods. Male Wistar rats were stratified into three metabolic models: (1) Chronic High-Fat Diet (HFD, 150-180 days) mimicking the SIRD phenotype; (2) HFD combined with low-dose streptozotocin (STZ, 30 days post-induction) mimicking the SIDD phenotype; and (3) HFD with supplemental glucose loading. Systemic phenotyping included longitudinal profiling of lipids, liver function, and protein status. Retinal cytoarchitecture was quantified using a computational pathomics pipeline (QuPath/StarDist) with deep learning-based segmentation of >90,000 nuclei, analyzed via Bayesian hierarchical modelling.

Results. The HFD model exhibited a "lipotoxicity-first" trajectory where dyslipidemia and hepatic steatosis preceded hyperglycemia. This SIRD-like phenotype was characterized by progressive retinal atrophy, specifically an 18-46% reduction in Inner Nuclear Layer neuronal density, accompanied by nuclear hyperchromasia indicative of epigenetic condensation. In contrast, the HFD+STZ model, characterized by acute hyperglycemia and significant hypoproteinemia, exhibited a paradoxical 1.54-fold increase in Ganglion Cell Layer density and thickening. This SIDD-like signature reflected cytotoxic edema driven by osmotic and oncotic failure, confirmed morphologically by severe irregular karyomegaly and profound chromatin dissolution (hypochromasia).

Conclusions. Retinal neurodegeneration is not a monolithic process but mirrors specific systemic metabolic trajectories. Chronic insulin resistance (SIRD) drives progressive atrophy via lipotoxicity, while insulin deficiency (SIDD) precipitates acute edema driven by hemodynamic oncotic failure. These findings establish retinal morphometry as a sensitive biomarker for systemic metabolic phenotyping, supporting a precision medicine approach to diabetic retinopathy.

Рецидивуюча ерозія рогівки (РЕР) — це хронічне захворювання епітелію рогівки, яке проявляється повторними епізодами раптового виникнення болю, як правило, в момент першого пробудження або вночі, що супроводжується почервонінням і рогівковим синдромом. Велике значення має пошук препаратів для скорочення терміну лікування, прискорення епітелізації та відновлення працездатності пацієнтів. Для відновлення структури рогівки використовують препарати, що впливають на епітеліальний шар, і перевагу надають очним мазям, які поліпшують метаболічні процеси й відновлюють будову епітелію. Метою роботи було підвищення якості лікування рецидивуючих ерозій рогівки після травматичних ушкоджень. Матеріали та методи. Під спостереженням перебували 9 пацієнтів (від 24 до 41 року) з РЕР різного ступеня тяжкості. Додатково до основної терапії була призначена офтальмологічна ізотонічна мазь Гілайс® Кеа з натрію гіалуронатом 0,4% на ніч упродовж місяця. Результати. Після проведеного лікування вже на 5-й день пацієнти відзначали суб’єктивне поліпшення симптомів (зменшення частоти виникнення різкого болю в момент пробудження, рогівкового синдрому й «туману перед оком»). Об’єктивно на 7-й день зменшувалась площа дефекту різною мірою залежно від початкових даних. Висновки. Включення в загальний комплекс лікування РЕР офтальмологічної ізотонічної мазі Гілайс® Кеа з натрію гіалуронатом 0,4% є доцільним завдяки її можливості довше утримуватися на очній поверхні й давати більш пролонгований і дієвий ефект, прискорювати епітелізацію рогівки, полегшувати суб’єктивний стан пацієнтів і поліпшувати якість життя таких хворих.

Recurrent corneal erosion (RCE) is a chronic disease of the corneal epithelium that manifests as recurrent episodes of sudden onset of pain, usually at first waking or at night, accompanied by redness and corneal syndrome. Of great importance is the search for medications that can shorten the treatment period, accelerate epithelialization, and restore patients’ ability to work. Among medicines used to recover the structure of the cornea, preparations that effect the epithelial layer are used, and preference is given to eye ointments, which improve metabolic processes and recover the structure of the epithelium. The purpose of the work was to improve the quality of treatment for recurrent corneal erosions after traumatic injuries. Materials and methods. Under observation, there were 9 patients (aged 24 to 41 years) with RCE of different degrees of severity. In addition to the main therapy, an ophthalmic isotonic ointment Gilays®Care with sodium hyaluronate 0.4% was prescribed before bed for a month. Results. After the treatment, already on day 5, patients noted subjective improvement of symptoms (reduction of the frequency of sudden pain on awakening, corneal syndrome and cloudy vision). Objectively, on day 7, the area of a defect of varying degree has decreased depending on the baseline data. Conclusions. The inclusion of ophthalmic isotonic ointment Gilays® Care with sodium hyaluronate 0.4% in the general treatment for RCE is expedient, due to the ability to stay longer on the ocular surface and provide a more prolonged and powerful effect to accelerate corneal epithelialization, improve the subjective condition of patients and their quality of life.
