The SARS-CoV-2 virus (severe acute respiratory syndrome coronavirus 2) has been showin to adversely affect kidney tissue, causing inflammatory, autoimmune, and metabolic disorders, which pose a risk of developing acute and chronic nephropathies. The lack of clear diagnostic criteria for kidney damage in children with post-COVID-19 syndrome (condition after recovering from COVID-19) complicates timely treatment and prevention of complications. Identifying early diagnostic markers of kidney damage is important for early detection of pathology, which would improve medical surveillance and timely treatment.Aim – to identify early diagnostic markers of kidney injury in children with post-COVID-19 syndrome.Materials and methods. This observational study included 30 children under the age of 18 diagnosed with post-COVID-19 syndrome who had documented urinary abnormalities during the acute phase of SARS-CoV-2 infection and were hospitalized between January 2023 and July 2024. The source of information was the «Medical Record of an Inpatient». The study was conducted using a targeted selective screening method. All children underwent comprehensive clinical and laboratory examinations according to generally accepted pediatric standards. The following indica-tors of kidney functional status were assessed: anti-crystallization capacity of urine (ACCU), daily salt excretion, lipid peroxidation (LPO) products, polar lipids (PL), and a series of urinalysis tests. The study results were compared with data from 40 children in the control group. Statistical analysis was performed using the EZR package (R-statistics).Results. It was found that the intensity of tissue hypoxia and dysfunction of the proximal nephron segments directly correlates with SARS-CoV-2-in-duced damage. This is evidenced by a decrease in ACCU towards calcium phosphates, reduced phosphate excretion, increased excretion of LPO products and appearance of PL, as well as a higher frequency of proteinuria and calciuria in urine compared to the control group.Conclusions. The possibility of predicting and diagnosing post-COVID-19 syndrome in children with urinary tract damage using non-invasive bio-chemical markers of tissue hypoxia and renal parenchymal membranolysis in children’s daily urine has been confirmed.The study was conducted in accordance with the principles of the Declaration of Helsinki. The research protocol was approved by the Local Ethics Committee of all institutions mentioned in the study. Informed consent was obtained from the parents and children.The authors declare no conflict of interest

Dysmetabolic nephropathy (DN) is one of the most common nephrological disorders in childhood, characterized by metabolic disturbances leading to intrarenal crystal formation and affecting 21‑31% of the pediatric population in Ukraine. This article presents the results of a comprehensive investigation into the genetic and epigenetic determinants of DN in children, including the clinical course, biochemical abnormalities, and molecular mechanisms of pathogenesis associated with different genotypes. The relevance of the study is underscored by the polygenic nature of DN, with up to 90% of cases exhibiting polygenic inheritance determined by complex
interactions between hereditary and environmental factors.
Objective. To characterize the genetic and epigenetic determinants of dysmetabolic nephropathy in children through analysis of polymorphisms in the VDR, GSTM1, and GSTT1 genes and assessment of early developmental factors.
Materials and methods. A total of 108 children aged 6‑17 years with DN and 44 healthy children from the Ivano-Frankivsk region were enrolled. Genotyping for VDR polymorphisms (TaqI, ApaI) and GSTM1/GSTT1 deletions was performed using polymerase chain reaction (PCR). A comprehensive clinical and laboratory evaluation was conducted to assess metabolic disorders, oxidative stress markers, and early developmental factors. The study was conducted in accordance with the principles of the Declaration of Helsinki.
The study protocol was approved by the Ethics Committee of Ivano-Frankivsk National Medical University (Protocol No. 145/24 dated 20 March 2024). Statistical analyses were performed using the SPSS software package (IBM Corp., Armonk, NY, USA). Intergroup comparisons were carried out using the Student’s t-test, Mann–Whitney U test, and chi-squared (χ²) test as appropriate. Odds ratios (OR) with 95% confidence intervals (CI) were calculated. The study was supported by the research program of the State Institution «Institute of Hereditary Pathology of the National Academy of Medical Sciences of Ukraine»: «Search for markers of early diagnosis and
differential diagnosis of congenital malformations associated with undifferentiated connective tissue dysplasia» (State Registration No. 0114U001549), and by the research programs of Ivano-Frankivsk
National Medical University: «Features of psychosomatic adaptation in children with chronic somatic pathology» (State Registration No. 0112U004423; implementation period 2012‑2021) and «Health status and adaptation features of children of Prykarpattia with somatic diseases, their prevention» (State Registration No. 0121U111129;
implementation period 2021‑2026).
Results. In children with dysmetabolic nephropathy (DN), significant associations with VDR gene polymorphisms were observed: the heterozygous Aa genotype (ApaI) was associated with a 5.69‑fold increase in DN risk (p < 0.001), and the Tt genotype (TaqI) with a 2.66‑fold increase. The double deletion of GSTM1/GSTT1 conferred a 4.73‑fold increase in DN risk. Among epigenetic factors, the most significant associations were observed for threatened abortion (15.83% versus 8.93%), gestosis during the first half of pregnancy
(53.83% versus 28.57%), early artificial feeding (67.31% versus 14.28%), and frequent acute respiratory infections in childhood (71.15% versus 12.50%). Elevated markers of oxidative stress and disturbances in connective tissue metabolism were also documented in patients.
Conclusion. Dysmetabolic nephropathy in children exhibits a polygenic inheritance pattern with a substantial contribution from epigenetic factors. The identified genetic markers may be utilized for early identification of at-risk individuals, while modification of epigenetic risk factors may serve as a basis for preventive strategies.

It  should  be  noted  that  oxidative  stress,  as  a  universal  mechanism  of  tissue  hypoxia  at  the  cellular  level, accompanied by non-enzymatic free radical oxidation and accumulation of lipid peroxidation products (LPO) in the blood, has attracted particular interest among medical professionals in our time. Research in the last decade has shown that there are all reasons to consider the activation of free radical LPO as a nonspecifi c component of physiological and pathological reactions characterizing the stress of activation of homeostasis maintenance systems.The aim of stady to establish the relationship between the impact of LPO processes, antioxidant defense, and membrane destruction of renal epithelium in children with dysmetabolic nephropathy.Materials and methods. Two groups of examined children were formed from the examined group, including those  with  dysmetabolic  nephropathy  and  secondary  urinary  tract  infections:  Group  I  -  52  individuals  in  whom  dysmetabolic nephropathy was complicated by the superimposition of infl ammatory processes in the kidneys and urinary tract - complicated DN (I-UDN), and Group II - 56 children with uncomplicated course of DN (II-DN), (a total of 108 children). The control group consisted of 65 healthy children. In children of all groups, the indicator of LPO process activity and the indicator of catalase activity in blood and urine were determined as a mechanism for regulating the antioxidant system of the body.Results. Against the intensifi cation of lipid peroxidation and membrane destruction processes in the bodies of children  with  DN,  the  possibilities  of  antioxidant  protection  are  exhausted,  which,  in  turn,  leads  to  even  greater  intensity of the LPO process. The catalase activity indicator in urine is an informative, reliable, and sensitive marker not only for the result of the impact of epigenetic factors on a child’s body but also a prognostic marker for a more severe course of dysmetabolic nephropathy in children.Conclusions.The revealed facts allow us to assert that against the background of intensifi  cation of lipoperoxidation and  membrane  destruction  processes  in  the  body  of  children  with  DN,  the  possibilities  of  antioxidant  protection  are  depleted,  which  in  turn  leads  to  an  even  greater  intensity  of  the  process  of  lipid  peroxidation.  And  the  index  of  catalase  activity  in  urine  is  an  informative,  reliable  and  sensitive  marker  not  only  of  the  result  of  the  impact  of  epigenetic  factors  on  the  child’s  body,  but  also  a  prognostic  marker  of  a  more  severe  course  of  dysmetabolic  nephropathy in children.

Сoronavirus disease 2019 (COVID-19) and its long-term consequences have a significant negative impact on the lives of people around theworld. The long-term consequences of acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection are known as the post-COVID-19syndrome, the symptoms of which can persist for more than 12 weeks after the onset of the disease. Despite the measures already takento control and combat the disease, the problem of establishing the structure of post-COVID-19 syndrome, especially in children, remainsan important global problem for society.Aim — to determine the data on the structure of the incidence of children with post-COVID-19 syndrome.Materials and methods. We examined 505 children under 18 years of age with post-COVID-19 syndrome who were inpatients duringJanuary-July 2023. The source of information was the medical record of an inpatient (Form 003/O). The study was conducted by randomization.All sick children underwent a comprehensive clinical and laboratory examination in accordance with standard, generally accepted methods ofclinical, laboratory and instrumental examination in pediatrics. Statistical processing was performed in the EZR program package (R-statistics).Results. It was established that in children with post-COVID-19 syndrome, the smallest number were children under 3 years old (12.87%),and the largest number were children from 12 to 18 years old (31.69%). In children with post-COVID-19 syndrome, the most commonly affectedsensory (80.20%) and nervous (78.61%) systems were noted.Conclusions. The lesions of the nervous and sensory systems in children with post-COVID-19 syndrome are explained by the comorbidityof SARS-CoV-2 tropism to these systems and a sharp deterioration in the socioeconomic and psycho-emotional state of children withpost-COVID-19 syndrome. The distribution of the examined children by gender and age simultaneously revealed a progressive increasein the frequency of male children with post-COVID-19 syndrome with increasing age. Identification of a combination of these factors in childrenmay increase pediatricians' alertness to the occurrence of post-COVID-19 syndrome in children and will facilitate timely correction and diagnosisof post-COVID-19 syndrome.The study was conducted in accordance with the principles of the Declaration of Helsinki. The study protocol was approved by the Local EthicsCommittee of all the institutions mentioned in the work. Informed consent of parents and children was obtained for the study.The authors declare no conflict of interest.
