Abstract

Background. Diabetic retinopathy (DR) is increasingly recognized as a heterogeneous neurovascular disorder, yet preclinical models often fail to distinguish between the distinct pathophysiological clusters of type 2 diabetes. Recent clinical evidence demonstrates that patients with different metabolic phenotypes, specifically Severe Insulin-Resistant Diabetes (SIRD) versus Severe Insulin-Deficient Diabetes (SIDD), exhibit divergent patterns of retinal complications. However, the specific cytomorphometric signatures that distinguish these phenotypes at the cellular level remain poorly characterized.

Aim: to determine whether specific systemic metabolic phenotypes, SIRD versus SIDD, induce distinguishable cytomorphometric signatures of retinal neurodegeneration in experimental models.

Materials and Methods. Male Wistar rats were stratified into three metabolic models: (1) Chronic High-Fat Diet (HFD, 150-180 days) mimicking the SIRD phenotype; (2) HFD combined with low-dose streptozotocin (STZ, 30 days post-induction) mimicking the SIDD phenotype; and (3) HFD with supplemental glucose loading. Systemic phenotyping included longitudinal profiling of lipids, liver function, and protein status. Retinal cytoarchitecture was quantified using a computational pathomics pipeline (QuPath/StarDist) with deep learning-based segmentation of >90,000 nuclei, analyzed via Bayesian hierarchical modelling.

Results. The HFD model exhibited a "lipotoxicity-first" trajectory where dyslipidemia and hepatic steatosis preceded hyperglycemia. This SIRD-like phenotype was characterized by progressive retinal atrophy, specifically an 18-46% reduction in Inner Nuclear Layer neuronal density, accompanied by nuclear hyperchromasia indicative of epigenetic condensation. In contrast, the HFD+STZ model, characterized by acute hyperglycemia and significant hypoproteinemia, exhibited a paradoxical 1.54-fold increase in Ganglion Cell Layer density and thickening. This SIDD-like signature reflected cytotoxic edema driven by osmotic and oncotic failure, confirmed morphologically by severe irregular karyomegaly and profound chromatin dissolution (hypochromasia).

Conclusions. Retinal neurodegeneration is not a monolithic process but mirrors specific systemic metabolic trajectories. Chronic insulin resistance (SIRD) drives progressive atrophy via lipotoxicity, while insulin deficiency (SIDD) precipitates acute edema driven by hemodynamic oncotic failure. These findings establish retinal morphometry as a sensitive biomarker for systemic metabolic phenotyping, supporting a precision medicine approach to diabetic retinopathy.

Хронічна серцева недостатність із помірно зниженою або збереженою фракцією викиду лівого шлуночка залишається актуальною медико-соціальною проблемою через високу поширеність, складність діагностики, значний тягар коморбідності та несприятливий прогноз. У цьому консенсусі експертів узагальнено сучасні підходи до діагностики серцевої недостатності зі збереженою фракцією викиду лівого шлуночка, зокрема поетапний діагностичний пошук із використанням клінічної оцінки, ехокардіографічних показників і натрійуретичних пептидів. Окрему увагу приділено обмеженості доказової бази для більшості традиційних медикаментозних стратегій у пацієнтів із фракцією викиду лівого шлуночка ³ 40 % та потребі в додаткових патогенетично обґрунтованих підходах. Розглянуто роль патологічної гіперактивації мінералокортикоїдних рецепторів у розвитку запалення, фіброзу та ремоделювання міокарда. Представлено клінічні дані щодо фінеренону - інноваційного нестероїдного антагоніста мінералокортикоїдних рецепторів, який у дослідженні FINEARTS-HF продемонстрував зниження на 16 % ризику серцево-судинної смерті та подій, пов’язаних із погіршенням серцевої недостатності, в пацієнтів із фракцією викиду лівого шлуночка ³ 40 % при сприятливому профілі безпечності. Обґрунтовано доцільність включення фінеренону в майбутній стандарт сучасного лікування серцевої недостатності з помірно зниженою або збереженою фракцією викиду лівого

шлуночка в Україні.

Генітальний герпес є одним з найпоширеніших захворювань, що передаються статевим шляхом. Більшість випадків генітального герпесу діагностують у людей віком від 16 до 40 років, що корелює з більшою кількістю статевих контактів. Вірус простого герпесу 2-го типу (ВПГ-2) є основним вірусом, що спричинює генітальний герпес, хоча кількість випадків, спричинених ВПГ 1-го типу (ВПГ-1), зросла в останні десятиліття. Поширеність ВПГ-2 перевищує 50%, і це число зростає майже до 100% у групах ризику. Із урахуванням недостатньої, а подекуди й контраверсійної, інформації стосовно епідеміології, діагностики та лікування генітального герпесу на преконцепційному етапі та під час вагітності, метою даного огляду став аналіз джерел доказової медицини відповідно до розуміння та розв’язання проблем клінічного менеджменту генітального ВПГ під час проведення прегравідарної підготовки та під час вагітності. У наведеному огляді проаналізовані результати доклінічних та клінічних досліджень стосовно оптимальної діагностики та раціональної терапії герпесвірусної інфекції на прегравідарному етапі та під час вагітності із залученням найбільш перспективних препаратів. Виконано систематичний пошук даних по базах MEDLINE, ISI Web of Science, PubMed, Scopus, Google Scholar та Proquest за 2010–2023 рр. Для підтвердження діагнозу доцільно верифікувати ДНК ВПГ, за можливості, визначення його типу молекулярнобіологічними методами (ПЛР) у вмісті везикул, із дна виразок і біологічних рідин та у секретах організму з урахуванням локалізації процесу; при цьому рутинний скринінг на ВПГ усім вагітним не рекомендується. Для лікування первинного клінічного епізоду аногенітального герпесу як епізодичну терапію на етапі планування вагітності рекомендовано призначати перорально ацикловір, або валацикловір, або фамцикловір. Під час вагітності для лікування герпесвірусної інфекції препаратами вибору є ацикловір або валацикловір. Місцеве лікування не рекомендується через низьку ефективність. Генітальна інфекція ВПГ під час вагітності може призвести до інфікування плода або новонародженого. Ризик передачі новонародженому вищий, якщо інфікування серонегативної матері відбувається у ІІІ триместрі гестації. Щоденне вживання ацикловіру слід розглянути з 36 тиж вагітності, за відсутності інших акушерських показань до кесарева розтину слід передбачити вагінальні пологи. Жінки з первинним генітальним герпесом у ІІІ триместрі гестації мають високий ризик передачі ВПГ своїм новонародженим, тому їх необхідно проконсультувати та запропонувати кесарів розтин, щоб зменшити цей ризик. Фамцикловір входить до світових протоколів (США, Канада, Європа, Австралія) ефективного лікування герпетичної інфекції через високу біодоступність (77,0%) та найтриваліший період перебування в інфікованій вірусом клітині (до 20 год), ніж інші препарати для лікування ВПГ-2.

Genital herpes is one of the most common sexually transmitted diseases. Most cases of genital herpes are diagnosed in persons from 16 to 40 years old, which correlates with more sexual contacts. Herpes simplex virus, type 2 (HSV-2), is the main virus that causes genital herpes, although the number of cases caused by HSV type 1 (HSV-1) has increased in the recent decades. The prevalence of HSV-2 exceeds 50%, and this number rises to almost 100% in risk groups. Taking into account the insufficient, and sometimes controversial, information about the epidemiology, diagnosis and treatment of genital herpes at the preconception stage and during pregnancy, the purpose of this review was to analyze the sources of evidence-based medicine in accordance with the understanding and solution of the problems of clinical management of genital HSV during pre-gravid preparation and during pregnancy. This review analyzes the results of preclinical and clinical research on optimal diagnosis and rational therapy of herpes virus infection in the pre-gravid period and during pregnancy with the involvement of the most effective drugs. A systematic data search was performed using MEDLINE, ISI Web of Science, PubMed, Scopus, Google Scholar, and Proquest databases for 2010–2023. To confirm the diagnosis, it is advisable to verify HSV DNA, if possible, to determine its type by molecular biological methods (PCR) in the contents of vesicles, from the bottom of ulcers and in the biological fluids, organism secretions, taking into account the localization of the process; however, routine HSV screening is not recommended for all pregnant women. For the treatment of the primary clinical episode of anogenital herpes as an episodic therapy at the stage of pregnancy planning, oral acyclovir, or valacyclovir, or famciclovir is recommended. During pregnancy, the drugs of choice for the treatment of herpesvirus infection are acyclovir or valacyclovir. Local treatment is not recommended due to low efficiency. Genital HSV infection during pregnancy can lead to infection of the fetus or newborn. The risk of transmission to the newborn is higher if the infection of the seronegative mother occurs in the third trimester of pregnancy. Daily use of acyclovir should be considered from 36 weeks of pregnancy, in the absence of other obstetric indications for cesarean section, vaginal delivery should be planned. The women with primary genital herpes in the third trimester of pregnancy are at high risk of transmitting HSV to their newborns, so they should be counseled and offered caesarean section to reduce this risk. Famciclovir is included in the world protocols (USA, Canada, Europe, Australia) for the effective treatment of herpes infection due to its high bioavailability (77.0%) and the longest period of stay in the virus-infected cell (up to 20 hours) than other drugs for the treatment of HSV-2.
