Бронхолегенева дисплазія (БЛД) є найпоширенішим хронічним респіраторним захворюванням передчасно народжених немовлят, яке може призводити до довгострокового порушення функції легень, підвищеної схильності до респіраторних інфекцій, затримки неврологічного розвитку, зниженої толерантності до фізичного навантаження тощо. Запалення відіграє центральну роль в патогенезі БЛД. Глюкокортикоїди мають потужну протизапальну дію, пригнічуючи трансляцію прозапальних медіаторів та активуючи численні протизапальні механізми. Водночас, застосування глюкокортикоїдів може підвищувати частоту дитячого церебрального паралічу й інших порушень неврологічного розвитку, що визначає небезпеку їх рутинного використання та вимагає призначення схеми лікування, яка матиме найбільшу ефективність одночасно з найнижчими ризиками негативних побічних ефектів. У цьому огляді представлено сучасну стратегію оптимального постнатального застосування стероїдів, яка забезпечує максимальний ефект щодо профілактики БЛД та зменшує ризики небезпечних ускладнень.

Bronchopulmonary dysplasia (BPD) is the most common chronic respiratory disease of preterm infants. It can lead to long-term impairment of lung function, greater susceptibility to respiratory infections, delayed neurological development, and reduced tolerance to physical exertion. Inflammation plays a central role in the pathogenesis of BPD. Glucocorticoids have a strong anti-inflammatory effect. They suppress the translation of pro-inflammatory mediators and activate multiple anti-inflammatory mechanisms. However, glucocorticoid use can raise the incidence of cerebral palsy and other neurodevelopmental disorders. This risk makes routine use dangerous
and requires a treatment regimen that offers the greatest effectiveness with the lowest risk of negative side effects.
This review presents a modern strategy for postnatal steroid use. The strategy seeks maximum effect in preventing BPD and reducing the risk of dangerous complications.

Бронхолегенева дисплазія (БЛД) залишається поширеним хронічним респіраторним захворюванням передчасно народжених немовлят, яке може призводити до довгострокових порушень та мати тривалий вплив на якість подальшого життя. Водночас, клінічні ознаки й ускладнення цієї патології можуть відрізнятися навіть у пацієнтів із тими самими чинниками ризику в анамнезі. Це пов’язано з тим, що незрілі легені зазнають комбінованих потенційно шкідливих впливів, які відрізняються за природою, моментом дії, тривалістю і тяжкістю. Генетичні й епігенетичні чинники модулюють реакцію на початкове ураження легень, формуючи унікальну модель їх ушкодження та відновлення. Ступінь впливу патологічних чинників на розвиток альвеол, інтерстиційної тканини, центральних і дистальних дихальних шляхів, а також легеневих судин в популяції передчасно народжених немовлят з діагнозом БЛД також є неоднорідною. Багатофакторна етіологія БЛД й унікальні моделі ушкодження та відновлення незрілих легень реалізуються у різні клінічні фенотипи хвороби, які мають різні віддалені наслідки. Лікувальні утручання часто є неспецифічними, не враховують особливості фенотипу захворювання і тому мають недостатню ефективність. Отже,
встановлення фенотипів БЛД є важливим для кращої стратифікації ризиків і відповідно ефективнішого лікування. У цьому огляді представлено сучасні уявлення про фенотипи БЛД, своєчасна діагностика яких є основою ефективної допомоги дітям із цією патологією.

Bronchopulmonary dysplasia (BPD) represents a highly prevalent chronic respiratory disease affecting preterm infants, frequently resulting in persistent respiratory impairment and long-term sequelae. Even among infants presenting with similar perinatal risk factors, the clinical manifestations and complications of BPD exhibit considerable variability. Such variability reflects the complex interaction of multiple injurious factors – varying in nature, timing, duration, and severity – to which the immature lungs are exposed. Furthermore, genetic
and epigenetic factors modulate the response to initial lung injury, thereby yielding individualized patterns of pulmonary damage and repair. The extent to which pathological factors affect alveolar development, the interstitial compartment, proximal and distal airways, and the pulmonary vasculature similarly exhibits heterogeneity among preterm infants with BPD. This multifactorial pathogenesis, combined with individual patterns of injury and repair in the immature lung, contributes to the development of distinct clinical BPD phenotypes, each potentially associated with divergent long-term outcomes. Current therapeutic approaches remain largely nonspecific
and frequently fail to adequately account for this phenotypic heterogeneity, thereby limiting their clinical efficacy.
The identification and characterization of BPD phenotypes are thus essential for improving risk stratification and developing more individualized therapeutic strategies. This review summarizes current concepts regarding BPD phenotypes and their clinical characteristics, highlighting the importance of timely phenotype recognition as a basis for personalized and effective management of affected infants and children.

Покращення показників виживання надзвичайно недоношених немовлят не забезпечило зменшення частоти
бронхолегеневої дисплазії (БЛД), яка все ще діагностується у значної частки таких дітей. Незважаючи на наявність декількох ефективних методів профілактики цього захворювання, у частини передчасно народжених немовлят формуються тяжкі форми БЛД, і такі діти можуть потребувати тривалої ендотрахеальної штучної вентиляції легень (ШВЛ). Упродовж останніх років основна увага дослідників та клініцистів була зосереджена на профілактиці БЛД, а лікування пацієнтів зі встановленим тяжким захворюванням переважно було емпіричним. У цьому огляді представлено сучасну концепцію фізіологічно обґрунтованої стратегії ШВЛ у дітей зі встановленим діагнозом тяжкої БЛД, яка відрізняється від підходів, які традиційно використовують під час лікування респіраторного дистрес-синдрому.
Допомога немовлятам із тяжкою БЛД потребує застосування моделі лікування хронічного захворювання
й іншої мети лікувальних утручань порівняно з використанням стандартних підходів, які переважно практикують у відділеннях інтенсивної терапії новонароджених щодо пацієнтів з гострою патологією.

Improved survival rates for extremely preterm infants have not reduced the incidence of bronchopulmonary dysplasia (BPD), which is still diagnosed in a significant proportion of these infants. Despite the availability of several effective methods for preventing this disease, some of these infants develop severe forms of BPD, and such infants may require prolonged endotracheal mechanical ventilation. In recent years, the focus of researchers and clinicians has been on the prevention of BPD, and the management of patients with established severe disease has been largely empirical. This review presents a modern concept of a physiologically based strategy for
mechanical ventilation in infants with an established diagnosis of severe BPD, which differs from the approaches traditionally used in the management of respiratory distress syndrome. Care for infants with severe BPD requires a chronic disease model and a different focus of intervention than the standard approaches that are typically used in neonatal intensive care units for patients with acute conditions.

INTRODUCTION
Invasive mechanical ventilation (MV) remains a widely used respiratory support for the sickest very-low-birth-weight (VLBW) infants. However, prolonged exposure to this invasive treatment can be associated with adverse outcomes. It is essential to establish the factors which influence the duration of MV. The study aimed to determine the factors affecting the duration of MV in VLBW infants.
MATERIALS AND METHODS
Data obtained from a prospectively created computer database were used in a retrospective cohort study. The database included information about 1,086 VLBW infants < 32 weeks of gestation who were intubated and mechanically ventilated at any time during their hospital stay at Lviv Regional Clinical Hospital between January 2010 and December 2020.
RESULTS
The infants had a mean (SD) gestational age of 27.6 (2.2) weeks and birth weight of 1,007 (262) g. 43% of them were delivered by cesarean section, 26% from multiple pregnancies, 58% were intubated and ventilated at birth, and 49% were treated with surfactant. Severe intraventricular hemorrhages (IVH) occurred in 179 (16%), periventricular leukomalacia (PVL) in 60 (6%), bronchopulmonary dysplasia (BPD) in 135 (12%), and necrotizing enterocolitis (NEC) in 41 (4%) infants. In 49 (5%) cases, the BPD was severe. 678 (62%) patients survived until discharge. The median (IQR) duration of endotracheal MV was 47 (10-103) hours. BPD (rS = 0.32, p < 0.05), severe BPD (rS = 0.418, p < 0.05), pneumothorax (rS = 0.06, p = 0.05), severe IVH (rS = 0.255, p < 0.05), PVL (rS = 0.15, p < 0.05), sepsis (rS = 0.087, p < 0.05), NEC (rS = 0.088, p < 0.05), antibiotic therapy duration (rS = 0.168, p < 0.05), and smaller gestational age (rS = -0.118, p < 0.05) were reliably associated with longer duration of MV in VLBW infants.
Based on a one-way analysis of covariance, only severe BPD (F = 20.898, p < 0.0001) and PVL (F = 5.989, p < 0.05) significantly and independently increased the duration of MV.
CONCLUSIONS
Severe lung injury and brain injury are the main factors affecting the duration of MV in our 10-year cohort of VLBW infants.

Бронхолегенева дисплазія (БЛД) є провідною хронічною патологією передчасно народжених дітей, яка змінює структуру легень та порушує розвиток легеневих судин. Найважливішим серцево-судинним ускладненням БЛД є розвиток легеневої гіпертензії, який діагностують у близько 25 % найтяжче хворих немовлят. Легенева гіпертензія, пов'язана з БЛД, виникає на тлі аномальної будови і зменшення кількості легеневих судин, що призводить до підвищення їх резистентності та розвитку правошлуночкової серцевої недостатності. Виникнення цього ускладнення погіршує прогноз виживання у дітей з БЛД, подовжує тривалість перебування у стаціонарі, негативно впливає на довгостроковий соматичний та неврологічний розвиток і збільшує частоту повторних госпіталізацій. Усе це обґрунтовує необхідність своєчасної діагностики та лікування легеневої гіпертензії у дітей з БЛД. У цьому огляді представлені нові дані щодо визначення, діагностики та лікування легеневої гіпертензії, пов’язаної з БЛД.

Bronchopulmonary dysplasia (BPD) is a leading chronic pathology of premature infants, which changes the structure of the lungs and disrupts the development of pulmonary vessels. The most important cardiovascular complication of BPD is the development of pulmonary hypertension, which is diagnosed in about 25 % of severely ill infants. Pulmonary hypertension associated with BPD develops due to lung vascular abnormalities and remodeling of the pulmonary vasculature, both of which lead to an increase in vascular resistance and the development of right ventricular heart failure. The occurrence of this complication worsens the prognosis of survival in infants with BPD, prolongs the total duration of hospital stay, adversely affects long-term somatic and neurological development and increases the frequency of re-hospitalizations. All this justifies the need for timely diagnosis and treatment of pulmonary hypertension in children with BPD. This review presents new data, for the definition, diagnosis, and treatment of pulmonary hypertension associated with BPD.
