Objective: The aim: The aim of this work is to describe the clinical case of the first lung transplant in Ukraine through anterolateral access and to analyze management of patient in the postoperative period, followed by assessment of his general condition and quality of life.

Patients and methods: Materials and methods: Analysis of the clinical case of the patient who underwent the first lung transplantation in Ukraine, including a description of surgery, anesthesia, and postoperative recovery.

Conclusion: Conclusions: Thus, based on the data of the world literature, as well as on the results of treatment on the basis of Municipal Non-Profit Enterprise " Lviv Emergency Clinical Hospital" we can conclude that lung transplantation is one of the most effective methods of treatment of patients with end-stage lung disease. This operation improves patients' quality of life, general well-being and ability to physical activity.
