Бронхолегенева дисплазія (БЛД) залишається поширеним хронічним респіраторним захворюванням передчасно народжених немовлят, яке може призводити до довгострокових порушень та мати тривалий вплив на якість подальшого життя. Водночас, клінічні ознаки й ускладнення цієї патології можуть відрізнятися навіть у пацієнтів із тими самими чинниками ризику в анамнезі. Це пов’язано з тим, що незрілі легені зазнають комбінованих потенційно шкідливих впливів, які відрізняються за природою, моментом дії, тривалістю і тяжкістю. Генетичні й епігенетичні чинники модулюють реакцію на початкове ураження легень, формуючи унікальну модель їх ушкодження та відновлення. Ступінь впливу патологічних чинників на розвиток альвеол, інтерстиційної тканини, центральних і дистальних дихальних шляхів, а також легеневих судин в популяції передчасно народжених немовлят з діагнозом БЛД також є неоднорідною. Багатофакторна етіологія БЛД й унікальні моделі ушкодження та відновлення незрілих легень реалізуються у різні клінічні фенотипи хвороби, які мають різні віддалені наслідки. Лікувальні утручання часто є неспецифічними, не враховують особливості фенотипу захворювання і тому мають недостатню ефективність. Отже,
встановлення фенотипів БЛД є важливим для кращої стратифікації ризиків і відповідно ефективнішого лікування. У цьому огляді представлено сучасні уявлення про фенотипи БЛД, своєчасна діагностика яких є основою ефективної допомоги дітям із цією патологією.
Bronchopulmonary dysplasia (BPD) represents a highly prevalent chronic respiratory disease affecting preterm infants, frequently resulting in persistent respiratory impairment and long-term sequelae. Even among infants presenting with similar perinatal risk factors, the clinical manifestations and complications of BPD exhibit considerable variability. Such variability reflects the complex interaction of multiple injurious factors – varying in nature, timing, duration, and severity – to which the immature lungs are exposed. Furthermore, genetic
and epigenetic factors modulate the response to initial lung injury, thereby yielding individualized patterns of pulmonary damage and repair. The extent to which pathological factors affect alveolar development, the interstitial compartment, proximal and distal airways, and the pulmonary vasculature similarly exhibits heterogeneity among preterm infants with BPD. This multifactorial pathogenesis, combined with individual patterns of injury and repair in the immature lung, contributes to the development of distinct clinical BPD phenotypes, each potentially associated with divergent long-term outcomes. Current therapeutic approaches remain largely nonspecific
and frequently fail to adequately account for this phenotypic heterogeneity, thereby limiting their clinical efficacy.
The identification and characterization of BPD phenotypes are thus essential for improving risk stratification and developing more individualized therapeutic strategies. This review summarizes current concepts regarding BPD phenotypes and their clinical characteristics, highlighting the importance of timely phenotype recognition as a basis for personalized and effective management of affected infants and children.
