Dysmetabolic nephropathy (DN) is one of the most common nephrological disorders in childhood, characterized by metabolic disturbances leading to intrarenal crystal formation and affecting 21‑31% of the pediatric population in Ukraine. This article presents the results of a comprehensive investigation into the genetic and epigenetic determinants of DN in children, including the clinical course, biochemical abnormalities, and molecular mechanisms of pathogenesis associated with different genotypes. The relevance of the study is underscored by the polygenic nature of DN, with up to 90% of cases exhibiting polygenic inheritance determined by complex
interactions between hereditary and environmental factors.
Objective. To characterize the genetic and epigenetic determinants of dysmetabolic nephropathy in children through analysis of polymorphisms in the VDR, GSTM1, and GSTT1 genes and assessment of early developmental factors.
Materials and methods. A total of 108 children aged 6‑17 years with DN and 44 healthy children from the Ivano-Frankivsk region were enrolled. Genotyping for VDR polymorphisms (TaqI, ApaI) and GSTM1/GSTT1 deletions was performed using polymerase chain reaction (PCR). A comprehensive clinical and laboratory evaluation was conducted to assess metabolic disorders, oxidative stress markers, and early developmental factors. The study was conducted in accordance with the principles of the Declaration of Helsinki.
The study protocol was approved by the Ethics Committee of Ivano-Frankivsk National Medical University (Protocol No. 145/24 dated 20 March 2024). Statistical analyses were performed using the SPSS software package (IBM Corp., Armonk, NY, USA). Intergroup comparisons were carried out using the Student’s t-test, Mann–Whitney U test, and chi-squared (χ²) test as appropriate. Odds ratios (OR) with 95% confidence intervals (CI) were calculated. The study was supported by the research program of the State Institution «Institute of Hereditary Pathology of the National Academy of Medical Sciences of Ukraine»: «Search for markers of early diagnosis and
differential diagnosis of congenital malformations associated with undifferentiated connective tissue dysplasia» (State Registration No. 0114U001549), and by the research programs of Ivano-Frankivsk
National Medical University: «Features of psychosomatic adaptation in children with chronic somatic pathology» (State Registration No. 0112U004423; implementation period 2012‑2021) and «Health status and adaptation features of children of Prykarpattia with somatic diseases, their prevention» (State Registration No. 0121U111129;
implementation period 2021‑2026).
Results. In children with dysmetabolic nephropathy (DN), significant associations with VDR gene polymorphisms were observed: the heterozygous Aa genotype (ApaI) was associated with a 5.69‑fold increase in DN risk (p < 0.001), and the Tt genotype (TaqI) with a 2.66‑fold increase. The double deletion of GSTM1/GSTT1 conferred a 4.73‑fold increase in DN risk. Among epigenetic factors, the most significant associations were observed for threatened abortion (15.83% versus 8.93%), gestosis during the first half of pregnancy
(53.83% versus 28.57%), early artificial feeding (67.31% versus 14.28%), and frequent acute respiratory infections in childhood (71.15% versus 12.50%). Elevated markers of oxidative stress and disturbances in connective tissue metabolism were also documented in patients.
Conclusion. Dysmetabolic nephropathy in children exhibits a polygenic inheritance pattern with a substantial contribution from epigenetic factors. The identified genetic markers may be utilized for early identification of at-risk individuals, while modification of epigenetic risk factors may serve as a basis for preventive strategies.

It  should  be  noted  that  oxidative  stress,  as  a  universal  mechanism  of  tissue  hypoxia  at  the  cellular  level, accompanied by non-enzymatic free radical oxidation and accumulation of lipid peroxidation products (LPO) in the blood, has attracted particular interest among medical professionals in our time. Research in the last decade has shown that there are all reasons to consider the activation of free radical LPO as a nonspecifi c component of physiological and pathological reactions characterizing the stress of activation of homeostasis maintenance systems.The aim of stady to establish the relationship between the impact of LPO processes, antioxidant defense, and membrane destruction of renal epithelium in children with dysmetabolic nephropathy.Materials and methods. Two groups of examined children were formed from the examined group, including those  with  dysmetabolic  nephropathy  and  secondary  urinary  tract  infections:  Group  I  -  52  individuals  in  whom  dysmetabolic nephropathy was complicated by the superimposition of infl ammatory processes in the kidneys and urinary tract - complicated DN (I-UDN), and Group II - 56 children with uncomplicated course of DN (II-DN), (a total of 108 children). The control group consisted of 65 healthy children. In children of all groups, the indicator of LPO process activity and the indicator of catalase activity in blood and urine were determined as a mechanism for regulating the antioxidant system of the body.Results. Against the intensifi cation of lipid peroxidation and membrane destruction processes in the bodies of children  with  DN,  the  possibilities  of  antioxidant  protection  are  exhausted,  which,  in  turn,  leads  to  even  greater  intensity of the LPO process. The catalase activity indicator in urine is an informative, reliable, and sensitive marker not only for the result of the impact of epigenetic factors on a child’s body but also a prognostic marker for a more severe course of dysmetabolic nephropathy in children.Conclusions.The revealed facts allow us to assert that against the background of intensifi  cation of lipoperoxidation and  membrane  destruction  processes  in  the  body  of  children  with  DN,  the  possibilities  of  antioxidant  protection  are  depleted,  which  in  turn  leads  to  an  even  greater  intensity  of  the  process  of  lipid  peroxidation.  And  the  index  of  catalase  activity  in  urine  is  an  informative,  reliable  and  sensitive  marker  not  only  of  the  result  of  the  impact  of  epigenetic  factors  on  the  child’s  body,  but  also  a  prognostic  marker  of  a  more  severe  course  of  dysmetabolic  nephropathy in children.

Пошук літератури проводився в базі даних PubMed та Google Scholar з використанням наступних ключових слів: SARS-CoV-2 або COVID-19 та post-COVID, Long COVID, Diabetes mellitus та In children.

Background. An urgent and problematic issue in medicine today, in addition to the acute manifestation of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection, are the consequences of coronavirus disease 2019 (COVID-19), the so-called post-COVID syndrome (PCS). Currently, leading medical research institutions around the world are studying the causes, frequency and symptoms of PCS in both adults and children. Aim: to review the literature on the incidence and manifestations of post-COVID syndrome in children in order to draw the attention of medical professionals to the problem of post-COVID syndrome and its symptoms from various body systems. Materials and methods. The literature search was conducted in the PubMed and Google Scholar databases using the following keywords: “SARS-CoV-2 or COVID-19” and “post-COVID”, “long COVID”, “diabetes mellitus” and “in children”. Ukrainian literature search was conducted in the Google Scholar database using the following keywords: “SARS-CoV-2 or COVID-19” and “post-COVID”, “diabetes mellitus” and “in children”. The authors of the article reviewed the titles and abstracts of the found articles to select relevant publications. Results. The article provides data from the literature on PCS in children: definition of post-COVID syndrome in children, incidence, possible causes, pathogenesis and risk factors for the development of PCS. Signs of somatic, psychological and endocrinological manifestations of PCS are also given. The greatest attention is paid to the onset of type 1 diabetes mellitus (T1DM) in children after suffering COVID-19. It is noted that the frequency of T1DM in the pediatric population in the post-COVID period has almost doubled, to 0.043 versus 0.025 %. Global rate of new cases of T1DM in children in 2020 grew to 32.39 per 100,000 children compared to 19.73 per 100,000 children in 2019. Probable causes of diabetes after COVID-19 are direct cytolysis of pancreatic β-cells affected by the virus, and autoimmune reaction. A clinical case of diabetes mellitus in a young child as a possible manifestation of the PCS is provided. Conclusions. 1. The problem of PCS with various clinical manifestations in children is relevant and quite common. 2. PCS can develop not only in children with acute manifestations of COVID-19, but also in children with asymptomatic course. 3. Along with the most frequent somatic and psychological manifestations of COVID-19 in children, endocrinopathy may occur, such as diabetes mellitus.
