Анотація.Ознакою нашого часу є інфекції різної етіології, які стали особливо небезпечними для людей та тварин з ослабленим природним імунітетом. Вони поширені в різних природних і створених людьми середовищах: від промислових умов і закінчуючи медичними приладами, де вони можуть мати як позитивний, так і негативний вплив. В огляді досліджуються питання ролі мікробних біоплівок, набуття бактеріями стійкості до речовин з антимікробною активністю в контексті здоров’я людини, її наслідки та альтернативні методи профілактики спрямовані на ці пружні структури. Зосереджено увагу на асоціації біоплівок із стійкістю до антибіотиків та біоцидів, чутливості до дезінфектантів та проблем профілактики їх утворення.

 

Resume.A sign of our time are infections of various etiologies, which have become especially dangerous for people and animals with weakened natural immunity. They are widespread in various natural and man-made environments: from industrial conditions to medical devices, where they can have both positive and negative effects. The review examines the role of microbial biofilms, the acquisition of resistance by bacteria to substances with antimicrobial activity in the context of human health, its consequences and alternative methods of prevention aimed at these resilient structures. The focus is on the association of biofilms with resistance to antibiotics and biocides, sensitivity to disinfectants, and problems of preventing their formation.

Гострий інфаркт міокарда з елевацією сегмента ST (ST-elevation myocardial infarction, STEMI), не 
зважаючи на проведення реваскуляризації міокарда з імплантацією стента в інфарктпов’язану коронар
ну артерію (ІПКА), продовжує супроводжуватись летальними наслідками, безпосередньою причиною 
яких доволі часто є аритмії, зокрема фібриляція шлуночків. 

Ризик виникнення небезпечних для життя ускладнень (зокрема фібриляції шлуночків (ФШ), раптової зупинки кровообігу) при гострому інфаркті міокарда з елевацією сегмента ST (ST-elevation myocardial infarction – STEMI) є достатньо високим. Близько 4,0–7,0 % пацієнтів із STEMI, яким прово-
дилось первинне черезшкірне коронарне втручання (ПЧКВ) із стентуванням інфарктасоційованої коронарної артерії (ІАКА), помирають на стаціонарному етапі, що, звичайно, є тривожним явищем і вимагає пошуку причин появи небажаних подій та шляхів їх попередження.

Vertebral compression fractures are often the initial sign of systemic osteoporosis. Early detection and targeted treatment are crucial to prevent additional skeletal damage. Methods: Ten patients with systemic osteoporosis and confirmed lumbar compression fractures were examined (mean age 63 ± 2 years; 8 women, 2 men). All fractures occurred before patients were aware of their osteoporosis. Laboratory tests included 25(OH)D, CRP, calcium, ionized calcium, and phosphorus. DXA scans were performed at the hip and lumbar spine. Initial results showed vitamin D levels at 18.7 ± 1.3 ng/mL (normal 30), CRP at 7.25 ± 1.1 mg/L (normal <5), phosphorus at 0.65 ± 0.07 mmol/L (normal 0.81), and ionized calcium at 0.81 ± 1.1 mmol/L (normal 1.1). Femoral neck T-score was −2.2 ± 0.1, Z-score −2.3 ± 0.1; lumbar spine T-score +2.5 ± 0.1 and Z-score +2.8 ± 0.1, reflecting degenerative artifacts. All patients received vitamin D 5600 IU, ibandronic acid 150 mg monthly, and Osteogenon two tablets twice daily for 60 days. Results. After six months, improvements were observed in clinical, laboratory, and densitometric assessments. Vitamin D levels normalized in 9 patients (33.2 ± 2.3 ng/mL). DXA showed positive changes: hip T-score increased to −1.9 ± 0.1 and Z-score to −1.8 ± 0.1; lumbar spine T-score rose to −1.7 ± 0.1 and Z-score to −1.6 ± 0.1. Conclusion: The combined therapeutic approach of vitamin D repletion, anabolic vitamin D repletion, anti-resorptive therapy with ibandronic acid, and adjunctive Osteogenon recting metabolic abnormalities and enhancing bone mineral density. These findings support the importance of comprehensive, multimodal management in patients with systemic osteoporosis and vertebral compression fractures.

A longitudinal study of 145 military personnel with neuropathic residual limb pain after combat-related amputation demonstrated clinically significant pain in 60% of patients five months after amputation, with symptomatic neuroma predominating. PTSD was one of the most important contributors to neuropathic postamputation pain. A stable moderate direct correlation was found between pain intensity and PTSD symptoms; combat-related PTSD was associated with a 1.9-fold increase in the probability of neuropathic residual limb pain. Rehabilitation should be based on clinical pain phenotyping and a multidisciplinary therapeutic strategy.
