Особи з ХХН мають підвищений ризик серцево-судинних захворювань. Куріння є добре відомим фактором ризику серцево-судинних захворювань. Однак ризик внаслідок куріння для осіб із встановленим захворюванням нирок – менш очевидний. Відносно невелика кількість спостережних досліджень вивчали вплив куріння на функціональний стан нирок серед пацієнтів з ГКС.

Мета – вивчити вплив куріння на функціональний стан нирок у хворих з гострим коронарним синдромом.

Матеріали і методи. На базі ВП «Лікарня Святого Пантелеймона» Центру серця і судин – відділення кардіології та реперфузійної терапії, обстежено 60 хворих з гострим коронарним синдромом та супутньою патологією нирок. Хворих розподілено на дві групи: І групу (основну) склали 30 пацієнтів, в анамнезі яких – куріння більше 20 років, ІІ групу (порівняння) склали 30 хворих, які не курили. У І групі – чоловіків 93,4 %, жінок – 6,6 %; у ІІ, відповідно, – 66 % та 33 %. Середній вік хворих І групи – (58,12±1,6 6) року; ІІ групи – (60,34±2,5) року (p˃0,05).

Результати. Артеріальний тиск у курців був вищий, а саме, для І групи систолічний артеріальний тиск становив (141,29±4,0) мм рт. ст.; для ІІ – (132,75±5,1) мм рт. ст. (p<0,05);  діастолічний артеріальний тиск для І групи – (87,74±2,9) мм рт. ст.; для ІІ групи – (79,58±3,2) мм рт. ст. (p<0,05). Частота серцевих скорочень в основній групі становить 80,90±2,8 уд/хв; у групі порівняння – 80,75±1,86 уд/хв (p<0,05). Для вивчення функціонального стану нирок було визначено ШКФ. У хворих, які палили більше 20 років, виявлено достовірно нижчий рівень ШКФ на 33,54 % (І група – 58,61±2,84; ІІ група – 78,27±2,87, p<0,01).

Висновки. Фактор куріння пов’язаний з достовірним зниженням ШКФ у хворих з ГКС.

Individuals with chronic kidney disease (CKD) have an increased risk of cardiovascular diseases. Smoking is a well-known risk factor for cardiovascular diseases; however, its impact on individuals with established kidney disease is less clear. Relatively few observational studies have investigated the effects of smoking on kidney function in patients with acute coronary syndrome (ACS).

Objective – To study the impact of smoking on kidney function in patients with acute coronary syndrome.

Materials and Methods: The study was conducted at the St. Panteleimon Hospital, Center for Heart and Vascular Diseases, Department of Cardiology and Reperfusion Therapy. A total of 60 patients with acute coronary syndrome and concomitant kidney pathology were examined. Patients were divided into two groups: Group I (main group) included 30 patients with a smoking history of over 20 years, and Group II (comparison group) included 30 non-smoking patients. In Group I, 93.4% were men and 6.6% were women, while in Group II, 66% were men and 33% were women. The mean age of patients in Group I was (58.12±1.66) years, and in Group II – (60.34±2.5) years (p>0.05).

Results: Blood pressure was higher in smokers. Specifically, in Group I, the systolic blood pressure was (141.29±4.0) mmHg, compared to (132.75±5.1) mmHg in Group II (p<0.05); diastolic blood pressure in Group I was (87.74±2.9) mmHg, compared to (79.58±3.2) mmHg in Group II (p<0.05). The heart rate in the main group was 80.90±2.8 bpm, while in the comparison group it was 80.75±1.86 bpm (p<0.05). To evaluate kidney function, the glomerular filtration rate (GFR) was measured. Patients who smoked for more than 20 years had a significantly lower GFR by 33.54% (Group I – 58.61±2.84; Group II – 78.27±2.87, p<0.01).

Conclusions: Smoking is associated with a significant decrease in GFR in patients with acute coronary syndrome.

У пацієнтів з хронічною хворобою нирок (ХХН) підвищення рівнів NT-proBNP та ST2 пов’язане з розвитком атеросклерозу. Незважаючи на широке використання цих біомаркерів в клінічній практиці, не достатньо досліджена їхня роль при поєднанні ураження серця, зокрема гострого коронарного синдрому (ГКС) та ХХН, враховуючи фактор куріння. Проаналізовано рівні NT-proBNP та ST2 у 89 пацієнтів з ГКС та ХХН, середній вік хворих становив 60,36±1,07 років, частка жінок складала 28,09% (n=25), частка чоловіків – 71,91% (n=64). Залежно від рівня ШКФ хворі розподілені у дві групи: І група – 29 осіб зі ШКФ <60 мл/хв/1,73 м2, ІІ група – 60 осіб зі ШКФ ≥60 мл/хв/1,73 м2. Хворих обох груп було розподілено у підгрупи за фактором куріння: Ік (n=11), ІІк (n=24) – курці та Інк (n=18), ІІнк (n=36) – некурці. Визначення біомаркерів проводили у 2 добу перебування хворих в стаціонарі. У І групі, тобто серед хворих зі зниженою ШКФ, порівняно з ІІ групою, визначено у 3,21 рази достовірно вищий рівень NT-proBNP (687,0±169,77 пг/мл (І) проти 215,1±56,86 пг/мл (ІІ), p<0,05) та незначно вищий рівень ST2 (p>0,05). У підгрупі Ік, порівняно з підгрупою ІІк, встановлено у 4,31 рази достовірно вищий рівень NT-proBNP (1032,1±317,21 пг/мл (Ік) проти 240,2±81,15 пг/мл (ІІк), p<0,01) та вищий рівень ST2 (p>0,05). У підгрупі Інк, порівняно з підгрупою ІІнк, виявлено вищий рівень NT-proBNP (p>0,05) та у 1,24 рази достовірно вищий рівень ST2 (36,20±2,86 нг/мл (Інк) проти 29,20±2,66 нг/мл (ІІнк), p<0,05). У підгрупі Ік, порівняно підгрупою Інк, визначено вищий рівень NT-proBNP (p>0,05) та у 1,53 рази достовірно вищий рівень ST2 (56,40±6,93 нг/мл (Ік) проти 36,80±3,67 нг/мл (Інк), p<0,05). У підгрупі ІІк, порівняно з підгрупою ІІнк, виявлено у 2,14 рази достовірно вищий рівень NT-proBNP (233,80±67,97 пг/мл (ІІк) проти 109,10±12,61 пг/мл (ІІнк), p<0,05) та у 1,55 рази достовірно вищий рівень ST2 (46,50±3,17 нг/мл (ІІк) проти 30,00±2,46 нг/мл (ІІнк), p<0,001). У групі пацієнтів зі ШКФ <60 мл/хв/1,73 м2 (І) реєструються достовірно вищі середні рівні NT-proBNP та ST2 порівняно з ІІ групою, незалежно від фактору куріння. Разом з цим, у пацієнтів курців виявлено достовірно істотно вищі рівні NT-proBNP та ST2 порівняно з некурцями, незалежно від рівня ШКФ. Однак поєднання стану зниженої ниркової функції та фактору куріння спричиняють найвище зростання як рівня NT-proBNP (1032,10±317,21 пг/мл), так і ST2 (56,40±6,93 нг/мл). In patients with chronic kidney disease (CKD), elevated levels of NT-proBNP and ST2 are associated with the progression of atherosclerosis. Despite the widespread use of these biomarkers in clinical practice, their role in the combination of heart damage, particularly acute coronary syndrome (ACS) and CKD, considering smoking status, remains insufficiently studied. Levels of NT-proBNP and ST2 were analyzed in 89 patients with ACS and CKD, with a mean age of 60.36±1.07 years; women accounted for 28.09% (n=25) and men – 71.91% (n=64). Based on the glomerular filtration rate (GFR), patients were divided into two groups: Group I – 29 patients with GFR <60 ml/min/1.73 m², and Group II – 60 patients with GFR ≥60 ml/min/1.73 m². Both groups were further divided into subgroups based on smoking status: Іk (n=11), ІІs (n=24) – smokers, and Іns (n=18), ІІнк (n=36) – non-smokers. Biomarkers were measured on the 2nd day of hospitalization. In Group I, patients with reduced GFR showed a 3.21-fold significantly higher NT-proBNP level compared to Group II (687.0±169.77 pg/ml (I) vs. 215.1±56.86 pg/ml (II), p<0.05) and a slightly higher ST2 level (p>0.05). In subgroup Іk compared to ІІk, NT-proBNP was 4.31 times significantly higher (1032.1±317.21 pg/ml (Іs) vs. 240.2±81.15 pg/ml (ІІs), p<0.01) and ST2 was also higher (p>0.05). In subgroup Інк compared to ІІнк, NT-proBNP levels were higher (p>0.05) and ST2 was 1.24 times significantly higher (36.20±2.86 ng/ml (Іns) vs. 29.20±2.66 ng/ml (ІІns), p<0.05). In subgroup Іk compared to Інк, NT-proBNP levels were higher (p>0.05), and ST2 was 1.53 times significantly higher (56.40±6.93 ng/ml (Іs) vs. 36.80±3.67 ng/ml (Іns), p<0.05). In subgroup ІІs compared to ІІns, NT-proBNP was 2.14 times significantly higher (233.80±67.97 pg/ml (ІІs) vs. 109.10±12.61 pg/ml (ІІns), p<0.05) and ST2 was 1.55 times significantly higher (46.50±3.17 ng/ml (ІІs) vs. 30.00±2.46 ng/ml (ІІns), p<0.001). In the group of patients with GFR <60 ml/min/1.73 m² (Group I), significantly higher mean NT-proBNP and ST2 levels were observed compared to Group II, regardless of smoking status. At the same time, smokers demonstrated significantly higher levels of NT-proBNP and ST2 compared to non-smokers, regardless of GFR level. However, the combination of reduced kidney function and smoking resulted in the highest levels of both NT-proBNP (1032.10±317.21 pg/ml) and ST2 (56.40±6.93 ng/ml).

With the onset of the Covid-19 pandemic, the dominant pathogen of acute respiratory infections (ARI) became the coronavirus. Cases of coronavirus disease are registered throughout the year, with increase in the incidence in the autumn-winter period. Since the fall of 2022, the appearance of the type A influenza virus has been noted, which caused a severe course of the flu in patients. The aim of our study was to compare the distribution of laboratory-confirmed cases of Covid-19 and influenza. From January 2022 to April 2023, 4,205 nasopharyngeal swab specimens from Lviv Regional Infectious Disease Hospital inpatients with suspected ARI were examined by PCR. Also, from November 2022 to April 2023, 392 samples from the above-described patients were also examined for influenza. Covid-19 was laboratory confirmed in 1238 cases (29.5%). The age of the patients ranged from 2 to 94 years. One of the highest levels of laboratory-confirmed cases was recorded in January 2022 where 309 positive out of 578 collected cases (53.5%), with a decrease to 10% in June. An increase of morbidity was observed in August (46.8%) and September (56.5%). Since October, a decrease in the indicator (32.2%) was registered, followed by a drop in the level of laboratory-confirmed cases of the coronavirus disease to 5.2% in November and 1.9% in December. Since January 2023, there has been an increase in incidence rates with a peak (39.5%) in February and a subsequent gradual decline to 17.6% in April 2023. However, since November, the number of confirmed cases of influenza has increased sharply to 53.4% and 35.3% in December. Hospitalized patients with influenza had a severe course of the disease. Three patients had positive tests for Coronavirus and influenza A at the same time. From January to April 2023, no laboratory-confirmed cases of influenza A were reported, which corresponds to the increase in the incidence of Covid-19. Thus, in the structure of ARI, along with the coronavirus, the type A influenza virus plays an active role, both in terms of the number of cases and the severity of the course. That is, it can be possible that these pathogens compete with each other
