Автоімунні захворювання щитоподібної залози (АІЗЩЗ) посідають одне з провідних місць серед ендо-кринної патології та характеризуються багатофактор-ним впливом на метаболічні процеси організму. Вони часто поєднуються зі змінами рівня вітаміну В12, що може ускладнювати клінічний перебіг і діагностику супутніх станів, таких як анемія, хронічна втома та ког-нітивні порушення. Тому дослідження взаємозв’язків між автоімунними тиреоїдними маркерами і вітамі-ном В12 є актуальним і має важливе клінічне значення. Мета роботи — оцінити кореляційні зв’язки рів-нів гормонів й автоімунних тиреоїдних маркерів із показниками вітаміну В12 у дорослих та підлітків із дисфункцією щитоподібної залози. Матеріали та методи. У дослідження було залу-чено 97 дорослих і 28 підлітків із дисфункцією щито-подібної залози (ЩЗ). Дорослих розподілили на три групи залежно від патології ЩЗ: група 1 — гіпотиреоз на тлі автоімунного тиреоїдиту (АІТ) (n = 33, медіана віку — 47 [38; 57] років), група 2 — післяопераційний гіпотиреоз (n = 33, медіана віку — 48 [43; 55] років), група 3 — хвороба Грейвса (ХГ), тиреотоксикоз (n = 31, медіана віку — 51 [49; 54] рік); підлітків — на дві групи: група 1 — гіпотиреоз на тлі АІТ (n = 16, медіана віку — 14 [11,8; 15,3] років), група 2 — ХГ, тиреотоксикоз (n = 12, медіана віку — 12 [10,8; 15,3] років). Усім пацієнтам проводили визначення рівнів тиреотропного гормону (ТТГ), вільного тироксину (Т4), антитіл до тиреоїдної пероксидази (АТ-ТПО), антитіл до тиреоглобуліну (АТ-ТГ), ціанокобаламіну. Результати. Медіана вмісту АТ-ТПО у дорослих пацієнтів становила 20,1 [6,4; 111,5] МО/ мл. У 47,76 % (35,98 — 59,67 %) із 67 дорослих, яким проводили це дослідження, показник перевищував норму. Титр АТ-ТГ у 57,69 % із 26 дорослих, яким проводили дослі-дження, був вище за норму (105,3 [19,0; 119,9] МО/ мл (від 2,69 до 248,39 МО/ мл). Медіана рівня вітаміну В12була значно нижчою за норму (200 — 835 пг/ мл) — 100,0 [63,4; 178,4] пг/ мл (від 1,89 до 993,43 пг/ мл). Його значення були статистично значущо більшими в жінок (117,0 [74,1; 201,6] пг/ мл), ніж у чоловіків (75,3 [43,8; 116,9] пг/мл, р = 0,01). Рівень вітаміну В12у 85,71 % підлітків був нижче за норму (1,8 [23,8; 120,1] пг/мл (від 0,67 до 417,9 пг/мл).Висновки. У дорослих пацієнтів рівень ціаноко-баламіну мав доведену позитивну кореляцію із вміс-том Т4 (r = +0,23; р < 0,05), АТ-ТГ (r = +0,55; р < 0,05) та негативну — із концентрацією ТТГ (r = –0,23; р < 0,05) і був вищим у жінок (r = +0,28; p < 0,05), що вказує на
потенційний взаємозв’язок між забезпеченістю віта-міном В12 і дисфункцією ЩЗ. У підлітків кореляції між В12 та тиреоїдними антитілами були слабкими й ста-тистично незначущими, але в 91,7 % дівчат рівень В12був нижчий за норму. Отримані результати свідчать про вікові та гендерні відмінності взаємозв’язків між вітаміном В12 й автоімунними маркерами ЩЗ.
Autoimmune thyroid diseases (ATDs) occupy one of the leading places among endocrine pathologies and are characterized by a multifactorial impact on the met-abolic processes of the body. They are often combined with changes in vitamin B12 levels, which can compli-cate the clinical course and diagnosis of concomitant conditions, such as anemia, chronic fatigue, and cogni-tive impairment. Therefore, the study of the relationship between autoimmune thyroid markers and vitamin B12is relevant and has important clinical significance.Objective to assess the correlation between hormone levels and autoimmune thyroid markers with vitamin B12 levels in adults and adolescents with thyroid dysfunction.Materials and methods. 97 adults and 28 adolescents with thyroid dysfunction were included in the study. Adults were divided into 3 groups depending on thyroid pathology: group 1 — hypothyroidism secondary to autoimmune thyroiditis (AT) (n = 33, 34.02 %, median age 47 [38; 57] years), group 2 — postoperative hypothyroidism (n = 33, 34.02 %, median age 48 [43; 55] years), group 3 — Graves’ disease (GD), hyperthyroidism (n = 31, 31.96 %, median age 51 [49; 54] years). Among adolescents, 2 groups were formed depending on the existing thyroid dysfunction: group 1 — hypothyroidism secondary to AT (n = 16, 57.14 %, median age 14 [11.8; 15.3] years) and group 2 — GD, hyperthyroidism (n = 12, 42.9 %, median age 12 [10.8; 15.3] years). All patients were tested for levels of thyroid-stimulating hormone (TSH), free thyroxine (T4), antibodies to thyroid peroxidase (TPO-Ab), antibodies to thyroglobulin (TG-Ab), and cyanocobalamin.Results. The median value of TPO-Ab in adult patients was 20.1 [6.4; 111.5] IU/ml and ranged from 2.3 IU/ml to 536.05 IU/ml. In 47.76 % (35.98 — 59.67 %) of the 67 adults studied, TG-Ab level exceeded the normal range. TG-Ab was above normal in 57.69 % (38.60 — 75.66 %) of 26. The median value of TG-Ab was 105.3 [19.0; 119.9] IU/mL (from 2.69 IU/mL to 248.39 IU/mL). The median value of vitamin B12 was significantly lower than normal (200 — 835 pg/mL) — 100.0 [63.4; 178.4] pg/ml (Min 1.89 pg/ml, Max 993.43 pg/ml. Its values were significantly higher in women (117.0 [74.1; 201.6] pg/ml) than in men (75.3 [43.8; 116.9] pg/ml, p = 0.01). The level of vitamin B12 in 85.71 (70.63 — 95.96) % of adolescents was below the normal range, which also correlates with its low median level: 1.8 [23.8; 120.1] pg/ml (from 0.67 pg/ml to 417.9 pg/ml).Conclusions. In adult patients, the level of cyanocobalamin had a proven positive correlation with fT4 (r = +0.23; p < 0.05), TG-Ab (r = +0.55; p < 0.05) and a negative correlation with TSH (r = –0.23; p < 0.05) and was higher in women (r = +0.28; p < 0.05), indicating a potential relationship between vitamin B12 supply and thyroid dysfunction. In adolescents, the correlations between B12 and thyroid antibodies (TG-Ab, TPO-Ab) were weak and statistically insignificant, but among girls, 91.7 % had B12 levels below normal. The results indicate age and gender differences in the relationships between vitamin B12 and autoimmune thyroid markers.
