Introduction. Elevated lipoprotein(a) has emerged as an important determinant of cardiovascular risk because of its roles in atherosclerotic disease, vascular inflammation, endothelial dysfunction, arterial stiffening, and vascular calcification. Increasing evidence suggests that the clinical significance of Lp(a) extends beyond atherosclerosis, as higher concentrations have also been linked to vascular remodeling and adverse cardiovascular outcomes in patients with arterial hypertension.

Aim. To evaluate the current scientific evidence on the role of lipoprotein(a) in vascular remodeling and its effects on endothelial function, arterial stiffness, and cardiovascular risk among patients with arterial hypertension.

Materials and methods. The MEDLINE database via PubMed was searched for articles published between 2016 and 2026 using a predefined set of keywords. The search yielded 86 articles, which were initially screened and assessed based on their titles and abstracts. After evaluating relevance, we selected 56 articles for detailed assessment. Finally, 31 articles were included in the review.

Results. Elevated Lp(a) concentrations are linked to multiple mechanisms that drive vascular remodeling and cardiovascular injury. A substantial part of Lp(a)'s pathogenic activity is attributed to oxidized phospholipids, which contribute to endothelial dysfunction, vascular inflammation, extracellular matrix remodeling, fibrosis, and vascular calcification. Research indicates associations between elevated Lp(a) levels and increased pulse pressure, reduced nocturnal blood pressure decline, arterial stiffness, vascular calcification, and hypertensive target-organ damage. These findings suggest that elevated Lp(a) may influence the development of a less favorable vascular profile in arterial hypertension and contribute to cardiovascular risk beyond that explained by traditional lipid markers. The development of RNA-based therapeutic approaches, including antisense oligonucleotides and small interfering RNA agents, has enabled substantial reductions in circulating Lp(a) levels and may provide new opportunities for individualized cardiovascular risk management.

Conclusions. Elevated lipoprotein(a) levels should be regarded as an important genetically determined factor in vascular remodeling and cardiovascular risk in patients with arterial hypertension. Increased lipoprotein(a) concentrations contribute to endothelial dysfunction, vascular inflammation, arterial stiffness, vascular fibrosis, and calcification through multiple mechanisms, predominantly mediated by oxidized phospholipids.

Хвороба Кавасакі (ХК) — це васкуліт невідомого походження, що супроводжується лихоманкою та слизово-шкірним синдромом і пе-реважно зустрічається в немовлят. ХК є найпоширенішою причиною набутих вад серця в дітей у розвинених країнах, що ускладнюється виникненням аневризм коронарних артерій у 25% нелікованих пацієнтів внаслідок васкуліту коронарних артерій. Мета — проаналізувати рекомендації Американської асоціації серця, Єдиного центру та доступу до педіатричної ревматології в Європі (SHARE), а також інструкції японських спеціалістів щодо сучасних підходів до діагностики та лікування хвороби Кавасакі.Клінічні дослідження демонструють, що цей показник знижується до ∼4% під час лікування внутрішньовенним імуноглобуліном. Хоча деякі нові дослідження свідчать про гірші результати, навіть попри лікування імуноглобуліном (особливо в немовлят у віці до 12 місяців). Складність і гетерогенність перебігу ХК, широка диференціальна діагностика та відсутність специфічного діагностично-го дослідження нерідко є серйозними перешкодами на шляху до встановлення діагнозу.З урахуванням деяких розбіжностей американських, європейських та японських інструкцій у лікуванні ХК вважаємо за доцільне дотри-муватися європейських рекомендацій, які найбільш схожі до американських та найкраще підходять для європейців.Автори заявляють про відсутність конфлікту інтересів.Ключові слова: хвороба Кавасакі, повна форма Кавасакі, неповна форма Кавасакі, тривала лихоманка, висип, аневризми коронар-них артерій, імуноглобулін, варфарин, діти.
Kawasaki disease (KD) is a vasculitis of unknown origin, accompanied by fever and mucocutaneous syndrome, predominantly found in infants. KD is the most common cause of acquired heart disease in children in developed countries, which is complicated by the occurrence of coro-nary artery aneurysms in 25% of untreated patients due to coronary artery vasculitis. The aim is to analyze the guidelines of the American Heart Association, the Single Hub and Access Point for Paediatric Rheumatology in Europe (SHARE), as well as Japanese instructions on modern approaches to the diagnosis and treatment of Kawasaki disease.Clinical studies show that this rate decreases to ∼4% during treatment with intravenous immunoglobulin. Although some new studies show worse outcomes despite immunoglobulin treatment, especially in infants under 12 months of age. The complexity and heterogeneity of the course of KD, a wide differential diagnosis and the lack of a specific diagnostic tests are often serious obstacles on the way to diagnosis.Taking into account some differences in American, European, and Japanese guidelines for the treatment of KD, we consider it appropriate to follow European guidelines, which are most similar to American ones and are best suited for Europeans.The authors declare no conflict of interest.Keywords: Kawasaki disease, complete Kawasaki disease, incomplete Kawasaki disease, prolonged fever, rash, coronary artery aneurysms, immunoglobulin, warfarin, children.

Актуальність. На сьогоднішній день в Україні налічується декілька мільйонів громадян з цереброваскулярною хворобою
(ЦВХ), що пов’язано з прогресуванням старіння населення, несприятливими економічними та екологічними умовами, нераці-
ональним харчуванням, гіподинамією. Проте, основною причиною розвитку ЦВХ є розлад кровотоку у речовині мозку, який
порушує механізм саморегуляції мозкового кровообігу, викликаючи кисневе голодування та негативно впливаючи на систем-
ний кровообіг. Тому проблема підвищення ефективості медичної і фізичної реабілітації пацієнтів з ЦВХ потребує вирішення
шляхом підбору комплексу дієвих відновних засобів з урахуванням її стадії і періоду захворювання, станом мозкового кровоо-
бігу, етапів реабілітації, що обумовлює актуальність напрямку дослідження.
Мета дослідження. Визначення морфофункціональних показників, стан центральної та церебральної гемодинаміки
обстежених пацієнтів з І–ІІ стадіями ЦВХ.
Методи дослідження. Теоретичний аналіз та узагальнення наукової літератури, аналіз даних історії хвороби та амбу-
латорних карток, лабораторні методи, інструментальні методи дослідження – ЕКГ, УЗДГ магістральних артерій голови
та шиї, методи математичної статистики. Предмет дослідження – структура та зміст комплексної програми медичної
та фізичної реабілітації пацієнтів з ЦВХ І та ІІ стадій, визначення її впливу на функціональний стан організму.
Результати дослідження. Наведено результати обстеження функціонального стану гемодинамічних показників по
магістральних артеріях у 100 хворих з ЦВХ, що дасть можливість раціонально скорегувати комплекс реабілітаційних захо-
дів, враховуючи індивідуальні особливості та стадії перебігу хвороби. Визначено морфофункціональні показники, стан цен-
тральної та церебральної гемодинаміки обстежених пацієнтів з ЦВХ на грунті атеросклерозу та гіпертонічної хвороби.
Висновок. Комплексна оцінка рівня структурних змін функціонального стану гемодинамічних показників по магістраль-
них артеріях голови пацієнтів з ЦВХ дасть можливість раціонально скоригувати комплекс реабілітаційних заходів, врахову-
ючи індивідуальні особливості, стадію перебігу хвороби та супутні захворювання.

Kawasaki disease (KD) is an acute systemic vasculitis that is the most common cause of acquired heart disease in children under 5 years of age with hyperthermia. Diagnosis of KD is a clinical challenge, given the wide range of clinical manifestations and similarities with many viral and bacterial diseases. Purpose - to describe a clinical case of refractory КD in a three-month-old girl with an emphasis on the importance of echocardiography and coronary angiography for the final verification of the disease, urgent initiation of treatment with minimal suspicion of КD. Clinical case. The article reports on a three-month-old patient with a difficult diagnosis of refractory form of КD. The disease debuted with hyperthermia, enterocolitis, obstructive bronchitis, and hepatoliver syndrome. The manifestations of skin exanthema were initially considered as an allergic dermatitis to the use of a cephalosporin antibiotic. Multisystemic inflammatory syndrome associated with SARS-CoV-2 infection was suspected. After the use of immunosuppressive therapy with mega-doses of dexamethasone and intravenous human immunoglobulin 2 g/kg/day for 3 days, clinical improvement was achieved. Subsequently, the haemogram showed an increase in neutrophilic hyperleukocytosis to the appearance of blast cells, hyperthrombocytosis and severe anaemia. A differentiation was made between a leukemic reaction, the debut of myeloproliferative disease, juvenile myelomonocytic leukaemia. Against the background of a rapid decrease in the number of leukocytes, the girl developed hyperthermia, migratory intermittent maculopapular rash, and foots edema. Laboratory findings included thrombocytosis and an increase in acute-phase parameters. Echocardiography revealed left ventricular dilation, a small amount of excess fluid in the pericardium, dilatation of the left coronary artery evenly to the bifurcation. Computed tomography revealed giant coronary artery aneurysms, which gave grounds to diagnose KD
